OMC Sports Physicals

Students planning to participate in Minnesota State High School League (MSHSL) athletics and activities during the 2026-27 school year must have a current Sports Qualifying Physical Examination on file before participating.

All incoming 7th-grade students planning to participate in athletics must have a valid MSHSL physical on file. In addition, families should be aware that MSHSL physicals expire three (3) years from the date of the examination. On a typical schedule, most student-athletes need updated physicals prior to their 7th-grade and 10th-grade years, although families should verify the expiration date of their student's current physical to ensure continued eligibility.

To assist families in meeting this requirement, Olmsted Medical Center (OMC) is offering sports physical appointments at the following time:

Date: Monday, July 27, 2026

Time: 8:00 a.m. – 4:30 p.m.

Location: OMC Owatonna Clinic 210 N. Oak Avenue Owatonna, MN

Cost: $25

Parents or guardians must be present for the appointment, and the parent and student sections of the MSHSL Sports Qualifying Physical Examination Form must be completed and signed prior to the examination.

Students should bring:

  • Their immunization record

  • Glasses or contact lenses, if worn

Families may also complete sports physical requirements during a regularly scheduled well-child visit with their healthcare provider. Please check with your insurance provider regarding coverage for well-child visits and sports physical examinations.

We encourage all families to review their student's physical status and schedule appointments early to ensure eligibility for participation in fall athletics and activities.
