As we prepare for the 2026–2027 school year, families are reminded that all student-athletes must have a current Minnesota State High School League (MSHSL) Sports Qualifying Physical Examination before participating in athletics. In Minnesota, sports physicals are valid for three years, so many returning athletes may already have a current physical on file.
If your student-athlete needs an updated physical, Mayo Clinic Health System is offering several convenient opportunities at nearby locations.

Before Your Appointment
To help your appointment go smoothly, Mayo Clinic Health System recommends that student-athletes:
Complete the required pre-visit questionnaire through the Mayo Clinic Patient Portal before arriving.
Bring the completed MSHSL Sports Qualifying Physical Examination Form, with the medical history section completed and signed by a parent or guardian.
Be accompanied by a parent or guardian if under 18 years of age.
Wear loose-fitting clothing, gym shorts, and tennis shoes, and bring eyeglasses or contact lenses if they are worn.
New for Waseca Families
Beginning with the 2026–2027 school year, families will upload a copy of their student's completed sports physical directly into Waseca Public Schools' new online athletic registration system. Once uploaded, the form will be reviewed and the physical date will be recorded to satisfy MSHSL eligibility requirements.
If your student already has a valid physical, simply upload a copy during the registration process. If your student receives a new physical this summer, upload the updated form before the start of the athletic season.
Completing your sports physical early and uploading it during registration will help ensure your student-athlete is cleared and ready for the first day of practice.
